If you have paid up front for medical services (doctor visits, emergency hospital expenses, lab tests, x-rays, etc.), you will need to submit a claim for review for reimbursement (receive money back).

*If you were a covered during the 2021-2022 school year, you must submit any claims provided they are less than 1 year old to Medavie Blue Cross within 90 days of your coverage terminating. if you do not submit your claims by the 90 day deadline, your claim will not be processed and no refund will be provided.

TIPS

Ensure you complete as much of the form as you can to avoid delays in processing. You must include your name, student ID number and contact information.
Include the invoice or receipt from your medical appointment with your claim form.
Include any medical notes, doctor notes, lab results or prescriptions to support your claim.
If mailing in your claim, be sure to take copies of your receipts and documents.
